(If you are looking for a form that begins with "I" (such as I-130 or I-539) you may download or order them via mail from Citizenship and Immigration Services in the Department of Homeland Security (CIS) forms page.)

An Affidavit of Eligibility for Marriage. This is needed for an American to marry in Korea.

A completed Report and Certificate of Marriage. This form is also needed for an American to marry in Korea.

Form DS-2029/SS-5 05-2006, Application for a Consular Report of Birth Abroad and Application for Social Security Number Card. You should also download a sample to help you prepare the form.
