Replacing a Lost or Stolen Passport
Please report the loss/theft of all U.S. passports to the local police and the U.S. Embassy or Consulate serving your area as soon as possible. If your reported lost or stolen passport is found after a replacement passport is issued to you, the original passport cannot be used for travel.

Please submit the following items:
- Passport application must be filled out using the Passport Wizard. Once completed, please print the application form including the Statement Regarding a Lost or Stolen Passport, but DO NOT sign. Applications filled out by hand will not be accepted.
- Police Report
Report the loss to the police station nearest to where you lost your passport and bring that report with you. - Proof of U.S. Citizenship
Original U.S. birth certificate, naturalization certificate, Consular Report of Birth Abroad or previous U.S. passport. If you don't have an original proof of citizenship, you can still submit an application for a replacement passport. - Proof of Identity
Driver's license, military I.D., etc. Please bring any photo identification that you have. - One passport photograph
Sized 2" X 2" (or 5 cm X 5 cm) with a white background taken within the last six months. - Fee: $135
- Applicants under age 16: U.S. regulations require the consent of both parents/legal guardians for the issuance of a U.S. passport to a minor child.
- If only one parent submits the application for a minor child, the absent parent is required to complete a notarized Parental Statement of Consent Form DS-3053 (PDF-27.8KB) to express their consent for the issuance of the new passport. All sections of the parental consent form must be completely filled out including the notary office information and the type of ID used for the notary process. The parent submitting the application materials to the U.S. Embassy must bring the notarized consent form along with a copy of the absent parent’s ID used during the notary.
- The minor child (applicant) must be present along with the parent(s).
- A return delivery courier slip (available at the Embassy)
Note: To receive notification regarding the status of your passport, please provide an email address on the passport application.
Processing takes about two weeks upon receipt of a completed application. A return delivery courier slip filled out by you will be used to return your passport. Please note that you will pay for this service in cash directly to the courier upon delivery.

- A photo booth is now available in the ACS waiting room! For 7,000 Korean Won or $7 U.S. Dollars two passport photos will be produced and ready in a matter of minutes. We hope this will make it easier for our American Citizen Services customers to complete their passport applications and other submissions with a minimum of hassle.
