U.S. Passports
First Time Adult Passport Applicants

Please submit the following items:
Form DS-11 Application for a U.S. Passport The online version of this form includes instructions on the back side written for use in the U.S. which do not apply to your application in Seoul. When completing a passport application, please include the city in the "Place of Birth" section. For persons born in the U.S., list the city and state of birth. For persons born outside the U.S., include the city and country of birth.
Proof of U.S. citizenship
Original U.S. birth certificate, Consular Report of Birth Abroad or Naturalization Certificate. If the submitted document is under a different name, you must provide original evidence of the change of name, such as a marriage certificate or a court order. All documents must be originals; no copies please.
Please Note: Hospitals in the U.S. sometimes issue a commemorative "certificate of birth" which often has the baby's footprints. This hospital-issued certificate is not proof of citizenship. Certified birth certificates are issued by the State or County government, not a hospital.
Proof of Identity
For example, a driver's license, military I.D., etc. Again, all documents must be originals; no copies please.
One passport photograph
Sized 2" X 2" (or 5 cm X 5 cm) with a white/light background. The photo must be 2 inches by 2 inches with the face size as specified. Your browser may not display the graphic above as the correct size.
Application Fee $85.00 (below age 16),
$100.00 (Age 16 and above). you can pay the fee in U.S. Dollars, Korean Won or
credit cards.
Application for no-fee passports by US military dependents must be submitted with typed form DD-1056 signed in blue ink and the applicant's or the sponsor's military orders.
All first-time passport applicants must appear in person at the Embassy please.
