U.S. Passports
Replacing a Lost or Stolen Passport
Applications for replacement passports must be made in person. Embassies and consulates abroad can no longer issue full-validity passports. All applications for passports are forwarded to the United States for processing and it takes from one week to ten days for the issued passport to return. Embassies and consulates abroad may issue limited validity passports in cases of verifiable emergency.
Our ability to issue passports on the day of application can be affected by a number of factors, and can only be considered on a case-by-case basis in response to a verifiable emergency. Replacement passports cannot be issued outside of normal business hours except in documented cases of true life-or-death emergencies. Please do not make any unchangeable travel plans until your passport has actually been issued.

Please submit the following items:

Form DS-11, Application for a U.S. Passport.
When completing a passport application, please include the city in the "Place of Birth" section. For persons born in the U.S., list the city and state of birth. For persons born outside the U.S., include the city and country of birth. Also, make sure to include your SSN. Please contact the Social Security Administration in Manila FBU.Manila@ssa.gov for any inquiries about your SSN. -
Police Report
Report the loss to the police station nearest to where you lost your passport and bring that report with you. -

Proof of U.S. Citizenship
U.S. birth certificate, naturalization certificate, Consular Report of Birth Abroad or previous U.S. passport. If you have original proof of your citizenship, bring it with you when you make the application. If you don't have original proof of citizenship, you can still make an application for a replacement passport.
Please Note: Hospitals in the U.S. sometimes issue a commemorative "certificate of birth" which often has the baby's footprints. This hospital-issued certificate is not proof of citizenship. Certified birth certificates are issued by the State or County government, not a hospital.

Proof of Identity
For example, a driver's license, military I.D., etc. Please bring any photo identification that you have. -
Children Age Under the Age of 16: The State Department requires the consent of both parents for the issuance of a U.S. passport to U.S. citizens age 15 and younger. There are several special requirements. Please carefully review the information on this web site before you apply for a passport for a child age 15 and younger.

Application Fee
$85.00 (under Age 16), $100.00 (Age 16 and above) in dollars or in won. You can pay the fee in U.S. Dollars, Korean Won or credit cards.
