Replacing a Lost or Stolen Passport
Applications for replacement passports must be made in person. Embassies and consulates abroad can no longer issue full-validity passports. All applications for passports are forwarded to the United States for processing and it takes from one week to ten days for the issued passport to return. Embassies and consulates abroad may issue limited validity passports in cases of verifiable emergency.
If you will be seeking same-day replacement of your passport because of emergency travel, please apply no later than 9:30 AM. Our ability to issue passports on the day of application can be affected by a number of factors, and can only be considered on a case-by-case basis in response to a verifiable emergency. Replacement passports cannot be issued outside of normal business hours except in documented cases of true life-or-death emergencies. Please do not make any unchangeable travel plans until your passport has actually been issued.

Form
DS-11, Application for a U.S. Passport.
The online version of this form includes instructions on the back side written for use in the U.S. which do not apply to your application in Seoul. When completing a passport application, please include the city in the "Place of Birth" section. For persons born in the U.S., list the city and state of birth. For persons born outside the U.S., include the city and country of birth.
Police Report
Report the loss to the police station or police box nearest to where you lost your passport. The Seoul police lost and found department is reachable at 02-299-1282

Proof of U.S. Citizenship
U.S. birth certificate, naturalization certificate, Consular Report of Birth Abroad or previous U.S. passport. If you have original proof of your citizenship, bring it with you when you make the application. If you don't have original proof of citizenship, you can still make an application for a replacement passport.
Please Note: Hospitals in the U.S. sometimes issue a commemorative "certificate of birth" which often has the baby's footprints. This hospital-issued certificate is not proof of citizenship. Certified birth certificates are issued by the State or County government, not a hospital.
Proof of Identity
For example, a driver's license, military I.D., etc. Please bring any photo identification that you have.
One Passport Photograph (5 cm by 5 cm, white/light background)
Application Fee
$85.00 (under Age 16), $100.00 (Age 16 and above) in dollars or in won. You can pay the fee in U.S. Dollars, Korean Won or
credit cards.
