| Replacing a Mutilated/Damaged Passport
It is difficult for our office to advise you about using your mutilated/damaged passport without actually seeing its condition. Immigration officials around the world will be more likely to scrutinize a damaged passport.
If you believe that you may encounter difficulties due to the condition of your passport, please submit the following items for a replacement passport:
-
Form DS-11, Application for a U.S. Passport. When completing a passport application, please include the city in the "Place of Birth" section. For persons born in the U.S., list the city and state of birth. For persons born outside the U.S., include the city and country of birth. Also, make sure to include your SSN. Please contact the Social Security Administration in Manila FBU.Manila@ssa.gov for any inquiries about your SSN.
